Consents & compliance (NOM-004)

A set of features inside the patient record that help you meet NOM-004 (Mexico) (clinical record) and COFEPRIS requirements: informed consents, electronic and handwritten signatures, vital signs and your clinic's sanitary details. Everything is tied to the patient and can be printed or saved as a PDF.

Available on every plan. It's not a separate module: these are tabs and fields inside the patient record.

Informed consent per procedure

One document per procedure (extraction, root canal, surgery…) that the patient authorizes, with a sequential folio (for example CI-2026-0001).

1. In the record, open the 🖊️ Consents tab and click ＋ New consent.
2. Pick a template (general, extraction, root canal, surgery/anesthesia) or "Custom": the text fills in automatically and you can edit it.
3. Write the procedure title, choose the doctor and adjust the text.
4. Click Create consent.

Each consent has ✍️ Patient and ✍️ Doctor buttons to sign; once signed they turn to ✓ signed in green. You can also 🖨️ print or 🗑 delete.

Electronic and handwritten signatures

These are two different safeguards that complement each other:

• Electronic signature (seal): generated automatically on save. It's a unique code (hash) of the content, with the date and who signed; if the document changed, the seal would no longer match. It proves the document wasn't altered. Nothing to do on your part.
• Handwritten signature: the real stroke of the patient or the doctor, drawn on screen with a finger or mouse inside the box that appears. It works on a tablet, phone or computer — no stylus or external pad needed.

Ideally you have both: the patient signs on screen and the system adds the seal.

Vital signs

In the record → History, inside 📋 Records per appointment, capture the Vital signs (optional) alongside the clinical note for the visit: BP, HR, RR, temperature, weight, height, SpO₂ and glucose. They're all optional and are saved for each visit.

Sanitary details (COFEPRIS)

Set up once the details the law requires on clinical documents. In Settings → clinic profile, section Sanitary details (COFEPRIS), enter:

• Operating notice number (COFEPRIS)
• Sanitary officer and their license

From then on they appear automatically on prescriptions, consents and the printed clinical history. Only the admin user can edit these details.

Print or save as PDF

Each consent and the clinical history open in a printable window with your clinic's logo and details, the captured signatures and the electronic seal at the bottom. From the browser dialog you choose print or save as PDF.

Who can use it

• Consents and prescriptions: Front desk, Doctor, Assistant and Admin.
• Clinical records and vital signs: Doctor, Assistant and Admin.
• Sanitary details (COFEPRIS): Admin only.
• The Lab role does not have access to these modules.
